Common Name:  None given.
Etymology: 
formoso after the type locality, Rio Formoso.
Major Synonyms:  None.
History:  The first specimen was collected in 1995 and the second in 1996.
   

Morphological Characteristics: 

Troglomorphic Characters:  Abdomen naked.

Eye Condition:  Reduced eyes not externally visible.

Pigmentation:  It is totally depigmented.

Scales:  Scaleless.

Swim (gas) bladder:

   

Distribution:  Brazil: Mato Grosso do Sul state: Município (county) de Bonito: Serra da Bodoquena: Buraco do Ducho cave (21o08' S, 56o28' W).
